Operating principle and advantages
The method of individual surgical intervention modeling in horizontal concomitant strabismus STRABO according to IE Aznauryan is used to calculate enhancing and weakening interventions on the oculomotor muscles.
For combined enhancing/weakening interventions in concomitant convergent strabismus, the determination of dosage is performed through biomechanical calculations of the enhancement value for the muscle at the side opposite to the operated eye deviation, taking into account the following anthropometric eye parameters:
- the objective angle of strabismus is determined by a known method using haploscopic devices, as well as using visual oculography;
- eye anterior-posterior axis length - determined by ultrasonic A-scanning;
- static eye refraction - determined by the study of the eye optical power under complete drug cycloplegia;
- interpupillary distance.
However, as a general rule, the enhancing intervention dosage at each step is no more than 8 mm. If the enhancing intervention dosage is 6 mm and more, an additional weakening intervention of horizontal rectus muscle at the side of operated eye deviation is performed. The dosage for the weakening intervention is also calculated automatically.
For weakening interventions in concomitant strabismus, the determination of dosage is performed through biomechanical mathematical calculation of the attenuation value for the muscle at the side corresponding to the operated eye deviation, taking into account the following anthropometric parameters:
- the objective angle of strabismus is determined by a known method using haploscopic devices, as well as using visual oculography;
- eye anterior-posterior axis length - determined by ultrasonic A-scanning;
- the length of the transverse axis of the eye is determined by ultrasonic B-scanning or MRI.
- The diameter of the cornea is determined by the OCT of the anterior segment.
The following parameters are calculated automatically:
- intervention dosage
- residual deviation
- the number of steps required to achieve orthotropy.
Moreover, the doctor has the opportunity to simulate the result of the intervention during the calculation step, and demonstrate the result clearly to the patient before the procedure starts.
